Thank you for expressing an interest in SPAL products. To expedite the approval process please follow the steps below:
- Complete the New Account Customer Form and fax it back or e-mail it to our office in order
to set you up as a new customer.

- Include a copy of your business license and/or Federal Tax ID.
- Complete the 3rd page with Facsimile Consent Form. You can elect not to receive written
communication from us but the form still needs to be filled out.
Please be advised that it takes approximately 7 business days to process a Net 30 days Account and 2 business days to process a
Credit Card and/or COD (Company check on delivery) account.
